Hi Stuart,
It looks like a fluke, a body fluke (Gyrodactylus). There is another fluke which is related called the gill fluke(Dactylogyrus), these have eye spots at the front of the body.
Most koi carry a few flukes with no detriment to their health, but when there are many, problems can arise. It is the damage they cause to the skin allowing fungus, bacteria or virus to invade.
The best treatment is Flubenol which does no harm to the filter bacteria.
As from last November any medications used in the treatment of fish, has to be Authorised by the Veterinary Medicines Directorate (VMD). under the Small Animal Exemption Scheme.
Kusuri now market their own brand of Flubenol called Fluke - M.
This is currently the only legally obtainable form of Flubendazole, for use in Aquatic systems.
This is a very reliable and safe treatment for flukes.
regards
Dave.
